Request Additional Seats for the Gold Medal Ceremony!!!

This request is NOT an RSVP to the Gold Medal Ceremony! Gold Medalists still must submit their RSVP to the National Office in order to be included in the ceremony. This form is simply to request additional seating at the Gold Medal Ceremony over the four seats already included in your original RSVP.

Extra seats to the Gold Medal Ceremony may be requested in writing by emailing gold@congressionalaward.org or by completing this online form only. YOU STILL MUST SUBMIT YOUR RSVP FORM TO THE NATIONAL OFFICE BY MAY 1ST!!!

Decisions on additional guest allowances will be made after May 19th!
