Request Additional Seats for the Gold Medal Recognition Dinner!!!

This request is NOT an RSVP to the Gold Medal Recognition Dinner! Gold Medalists still must submit their RSVP to the National Office in order to reserve their seat at the Recognition Dinner. This form is simply to request additional seating at the Gold Medal Recgonition Dinner over the two guest seats already included in your original RSVP. Additional seats for the event will cost $100 each and will still be limited in order to allow additional guests for other families.

Extra seats to the Gold Medal Recognition Dinner must be requested by emailing gold@congressionalaward.org or by completing this online form. YOU STILL MUST SUBMIT YOUR RSVP FORM TO THE NATIONAL OFFICE BY MAY 1ST!!!

Decisions on additional guest allowances will be made after May 19th!
